Frontier Bonded Provider Notifications Update Request Form

Local Wholesale Customers currently receiving Frontier Provider Notifications (aka Line Loss Reports) via electronic bonded method (EDI or UOM), may request updates by completing this form. Questions related to completing the form may be directed to Frontier.Connectivity.Management@ftr.com.

NOTE: Company OCNs must be registered for the Provider Notifications in the Customer Wholesale Portal (CWP) in order to receive the reports via bonded method. Provider Notifications for Company OCNs not registered in the CWP are not eligible for receipt via bonded method.
