Get Connected to Order Voice and UNE Services
To establish a relationship to order new Frontier Local Voice and UNE services or request changes to existing Local Voice and UNE services wholesale customers must:
- Complete and submit the Frontier Connectivity Access Form to request VFO or Bonded connectivity. (Additional forms may be required and will be provided based on your connectivity selections.)
- Complete and submit the CLEC Master Account Questionnaire Form to establish a Local Billing Profile.
- Complete and submit the Billing Feed Connectivity Form to request Billing Feeds. (Optional if Billing Feeds are desired)

Industry Codes are required to do wholesale business with Frontier.
Additional Form Required for Local Services:
CLECs ordering Local Services such as voice products, voice with broadband, unbundled loops, LNP or Standalone Directory must also complete and submit the CLEC Master Account Questionnaire Form (CMAQ). All completed forms must be returned according to the instructions on the form.
CLECs are required to have a valid interconnection Agreement in place prior to establishing connectivity and completing a CMAQ. For more information on Interconnection Agreements, please visit the Interconnection page.

Billing Feed ConnectivityWholesale customers requiring billing feeds such as Billing Data Tape, Data Usage Files or Meet Point Billing must complete the Billing Feed Connectivity Form and submit as instructed on the form. Incomplete forms will be returned with no action.
